A release of information or a written request with client or gaurdian's signature and date will need to be completed for the client file, even if asking for records for yourself.

Proof of identity may be requested to uphold best confidentiality practices.

Requests may take up to 30 days to process.

Information in the recorde may be redacted if there is concern for safety or well-being.

Our documentation is geared towards justifying billing and supporting treatment.

Clients are encouraged to review records with their provider to avoid any misperceptions or confusion within the documentation.
